An array of services designed to assist individuals with serious and persistent mental illness in maximizing independence and role recovery, while managing symptoms and maintaining overall independence.

 

Programs include:
  • Intensive Psychiatric Rehabilitation Services

  • Habilitation Services

  • Community Support Services

  • Supported Community Living

  • See Disability Services for DayHab (Community Integration), Pre-Vocational Habilitation, Supported Employment, and other HCBS programs

Where: 

In the counties of Davis, Des Moines, Henry, Jefferson, Keokuk, Lee, Wapello, and Washington. In addition to individual homes and sites scattered throughout communities, First Resources Corporation operates an eight (8)-unit apartment complex in Ottumwa designed specifically to provide affordable housing to individuals with chronic mental illness.

How it Works: 

Individuals who may benefit from community based mental health services are typically assessed by an interdisciplinary team and may receive one or more of the services based on identified need. Services are provided based on an individual treatment plan for a pre-determined number of hours that range from a few hours a month to several hours per day. Funding Source(s): Title XIX, County contracts.
