Disability Services:

These programs include a variety of home and community-based services. These services are rehabilitative and instructional for the purpose of developing independent living skills. Individual plans are developed and designed to assist each person to meet goals related to their own particular desires. First Resources Corp. is currently certified to provide services under the Intellectual Disability waiver, Physical Disability waiver, Brain Injury waiver and Habilitation waiver (diagnosis of chronic mental illness).

Supported Community Living (SCL) Services

SCL Services are provided on either a daily or hourly basis depending on the individual's needs. FRC offers these services in apartment settings, 24-hr ID and BI homes as well as habilitation homes. Individuals receive assistance and support within their home environment with priority given to the following issues: obtaining and maintaining safe and healthy living environments, personal care, community integration, accessibility within the community, and support with self-advocacy skills. These services are provided in 13 southeast Iowa counties.

Day Habilitation Services

This program is designed to support individuals in developing and maintaining skills needed for community integration and inclusion. Skill development includes physical, emotional, communication, socialization, behavior management, mobility, and other supports identified in the individual's plan. This program also focuses on peer interaction and appropriate social skills, while promoting independence. Day Habilitation services provided to people on the ID waiver in these areas: Bloomfield, Centerville, Fairfield, Ottumwa, and Sigourney. Day Habilitation Services provided to people on the Habilitation waiver for chronic mental illness are offered at the Ottumwa location.

Respite Services

These services are designed to provide a break for caregivers who support individuals in the family home. The services provide the individual with the support and supervision necessary to ensure their safety and well-being in a setting of the family's choosing.

Consumer Directed Attendant Care (CDAC) Services

CDAC is provided to adults and children in the home and community to assist the consumer with self-care tasks which the consumer would typically do independently if the consumer were otherwise able. These services include non-skilled services such as dressing, hygiene, meal preparation, cooking, housekeeping, medication management, communication, transportation, etc.
