FRC is currently certified to provide services under the Intellectual Disability Waiver, Physical Disability Waiver, Brain Injury Waiver, Iowa Vocational Rehabilitation Services (IVRS), and Home-Based Habilitation.

Residential Services
Counties Served: Appanoose, Davis, Des Moines, Henry, Jefferson, Keokuk, Marion, Mahaska, Monroe, Van Buren, and Wapello
A service that provides short-term, intensive residential support to adults (18+) living with a diagnosis of severe and persistent mental illness (SPMI) and co-occurring conditions, such as substance use disorder or intellectual disability, accompanied by at least three areas of significant functional impairments. This service operates 24/7/365, with multiple staff per shift, and focuses on working closely with individuals to support progress towards stabilization and transitioning to a lower level of care.

Referrals can be submitted by a Case Manager or other qualified entity.
A complete referral must include the following prior to being considered:

Intensive Residential Services Application

Diagnostic records confirming SPMI or co-occurring diagnosis

Documentation of approved level of care completed within the past 60 days: LOCUS Tier 6, InterRAI Tier 6

Standardized functional assessment completed within the past 60 days: Comprehensive Assessment & Social History (CASH)​

Processing Referrals: Once a complete referral is received, our team will begin processing it. Individuals must be able to admit to services within 28 days of referral acceptance.

Residential Care Facility (RCF)
Daily services are provided to individuals in a RCF setting operated by First Resources Corp. located in Keosauqua. Services range from 18-24 hours per day. Services aim to be short-term, focusing on mental health stabilization, with the goal to transition individuals successfully into community living.

Waiver and Habilitation Homes
Daily services are provided to individuals in a home setting operated by First Resources Corp. Daily services range from five to twenty-four hours per day. Individuals receive assistance and support including but not limited to maintaining a healthy and safe living environment, personal care/hygiene, meal preparation, community integration, advocacy, and medication adherence.

Services are available for individuals accessing Habilitation, Intellectual Disability Waiver, Brian Injury Waiver, or Physical Disability Waiver and are available in the following areas: Bloomfield, Burlington, Centerville, Fairfield, Knoxville, Sigourney, Oskaloosa, and Ottumwa.

Employment Services
Counties Served: Appanoose, Davis, Jefferson, Keokuk, Mahaska, Monroe, and Wapello
An evidence-based service to Supported Employment that focuses on competitive integrated employment for individuals with a chronic mental illness. IPS staff work closely with job seekers, their family, mental health treatment team, and Iowa Vocational Rehabilitation Services (IVRS), to ensure rapid job search and placement, in the workplace of the job seeker’s choosing that best aligns with their strengths, wants, needs, and career goals.

At First Resources Corp. we offer an array of employment services including small group, job coaching and follow-along, job development, retail services, and workplace readiness assessments. Hourly Services
Counties Served: Appanoose, Davis, Des Moines, Henry, Jefferson, Keokuk, Mahaska, Monroe, Van Buren, and Wapello
Services are provided to individuals who reside independently or with family members in the community. Service times vary based on the individuals need ranging from once a month to five times a week, from an hour to four hours at a time. Individuals receive assistance and support including but not limited to maintaining a healthy and safe living environment, personal care/hygiene, meal preparation, community integration, advocacy, budgeting, shopping, and medication adherence. Additionally, we offer Respite and Consumer Directed Attendant Care (CDAC) on occasion.

Day Habilitation Services
Counties Served: Appanoose, Davis, Jefferson, Keokuk, Mahaska, and Wapello
Day Habilitation
A service designed to support individuals to reach their highest level of independence by providing meaningful daytime activities, and connecting individuals to their communities in positive, inclusive ways. Services include but are not limited to life skills development, behavior/symptom management, community inclusion and participation, support with achieving personal goals, self-advocacy training, support with daily routines and planning, promote independence, and pre-vocational experiences.
